Integrative Healthcare Solutions (Dr. Caffery) does not accept all new patient inquiries. We only want to take on patients we feel like he will have the best chance of helping. If you are interested in becoming a new patient then you must complete the steps below to determine if you are eligible to become a patient.

Download, print, and complete the new patient intake forms ("Adult" for 13 years of age and older, "Child" for 12 years of age and younger). If you don't have the ability to download/print the paperwork you can contact the office and they can be mailed or faxed to you, (856) 888-1860.

Fax, mail, or e-mail (non-secure, at your discretion) the new patient intake forms to the office. If the forms are sent by e-mail they must be in pdf format, we will not accept photos. If you do not have the ability to scan the forms into a pdf, mail or fax them to the office.

- Once the new patient forms are received in full and reviewed, you will be notified as to whether Dr. Caffery feels you are a good candidate to become a patient. You will typically be notified within 3 business days.
If you have not received a response from us within 3 business days then it is likely that we never received your paperwork. Verify the e-mail address (copy and paste from above), check to make sure the fax went through, and/or call the office and verify.
